Medication dosing and high-risk drug error
Safety Before 16 After 8Wrong drug, dose, concentration or rate is given during anesthesia, pain relief or symptom control, particularly with opioids, neuromuscular blockers, local anesthetics and infusions, causing overdose, underdosing or harm.
Who or what is affected: Patients, anesthetists, nurses and pharmacists
Causes
Time pressure in theatre, look alike drug labels, interruptions and unfamiliar concentrations.
Consequences
Respiratory depression, hypotension, awareness under anesthesia, serious harm and legal action.
Existing controls
- Controlled drugs are counted and signed at each handover.
- Infusion pumps have dose limits for common high-risk drugs.
Control gaps
Double checks are skipped during emergencies and pump limits do not cover every drug.
Additional controls
- Prevent: Require independent double checks for high-risk infusions and labelled prepared syringes before use.
- Detect: Review infusion pump alarms and dose error reports each week.
